What this site collects.
This website does not set advertising cookies and does not load third-party tracking scripts. Images are embedded in the pages themselves, so nothing is requested from an external image host as you browse.
The referral and case forms
Information and files submitted through the referral form and the case-submission form are used only to evaluate and handle the case you are sending, and to contact you about it. They are not sold, rented or shared with third parties for marketing.
These forms are intended for dental professionals and will frequently carry protected health information. Send patient information only where you are authorized to do so and in accordance with your own obligations under HIPAA and any applicable state law. If a case involves information you would rather not put through a web form, call the office and another route will be arranged.
Patient records
Clinical records created once a patient is seen at the practice are protected health information and are handled under the practice's HIPAA Notice of Privacy Practices, which is a separate document available from the office.
Accessibility
This site is built to be usable with a keyboard and with assistive technology, and it honors reduced-motion settings. If you encounter something that does not work for you, please call the office and say so.
